APPLICATION REQUIREMENTS

 

REQUIREMENTS (ALL DOCUMENTS MUST BE ORIGINAL UNLESS OTHERWISE STATED AS “XEROX” OR “COPY”)

 

1.) Philippine Passport

·       Broken passport is not accepted.

·       Passports must be signed and must have at least two (2) blank visa pages.

 

2.) Visa Application Form

·       Application form should be filled out all items correctly, if item is not applicable, please fill in (N/A). Embassy may not accept application with blank item, no signature or no date in the form; may deny the application that is filled out incorrectly or has wrong information.

·       Do not use liquid paper as means for correction. Corrections must be crossed out with a double line only.

3.) Photo

·       Specs: 4.5cm x 3.5cm, with white background.

·       Photo must be taken within 6 months.

·       Please write applicant’s name and birth date on back side of photo.

·       Photo must be pasted on the application form.

4.) Birth Certificate ORIGINAL

·       Must be issued within one (1) year from PSA

·       If birth certificate is unreadable or has incomplete information, please also provide birth certificate issued by the Local Civil Registrar.

·       If birth certificate is “Late Registration”, please submit in addition, a Baptismal Certificate and School Record (Form 137) from high school or elementary and School Yearbook (if possible).

·       If there is no record in PSA, please submit a “Certificate of Non-Record” from PSA together with a birth certificate from the Local Civil Registrar.

 

5.) Marriage Certificate (if the applicant is married) ORIGINAL

·       Must be issued within one (1) year from PSA.

·       If there is no record in PSA, please submit a “Certificate of Non-Record” from PSA together with a marriage certificate from the Local Civil Registrar.

·       If marriage certificate is unreadable or has incomplete information, please also provide marriage certificate issued by the Local Civil Registrar.

 

6.) Daily Schedule in Japan (Taizai Yoteihyo)

 

7.) Proof of relationship

·       Documents such as pictures, phone call details, receipts, passport copies and explanation letter about relationship with the visa applicant.

 

8.) Invitation Letter (Embassy form) (Signature/Seal) ORIGINAL

·       If Invitation Letter is of personal format, it must be addressed to “To: (Ambassador/Consul-General) of Japan”

 

9.) Residence Certificate (Juminhyo) of Friend/Acquaintance in Japan and of the Guarantor.

·       Must be issued within three (3) months.

·       Indicate all family members without omission.

 

10.) If friend/acquaintance in Japan is foreign resident, please submit a copy of friend’s Residence Card or Special Permanent Resident certificate of relative in Japan (both sides).

11.) Bank Certificate (Personal) (must be issued within three (3) months) ORIGINAL

 

12.) Income Tax Return (Form 2316) clear Photocopy (latest).

13.) Guarantee Letter ORIGINAL

·       If Guarantee Letter is of personal format, it must be addressed to “To: (Ambassador/Consul-General) of Japan”

 

14.) Income Certificate (Shotoku Shomeisho) from City Hall, Tax Return Certificates with Gross Income (Nouzeishomeisho: form 2) from tax office, KAKUTEISHINKOKUSHO HIKAE or Bank Certificate (YOKIN ZANDAKA SHOMEISHO) ORIGINAL

·       Must be issued within three (3) months. Submission of Withholding slip (GENSEN-CHOSHUHYO) in lieu of (11) is unacceptable. Please note that tax certificates to be submitted must indicate gross income amount.

·       Guarantor’s Income Certificates etc. is important in measuring the guarantor’s capability to support the applicant therefore, submission of more than one document (e.g. Income Certificate and Bank Certificate) is allowed.

 

 

15.) Foreign Residents with long-term or permanent status in the Philippines applying for a visa must submit, in addition to the above requirements (except birth and marriage certificates)

·       A copy of their Alien Certificate of Registration (ACR) issued by the Philippine Government.

NOTICE:

Size of document for application should be A4 size only. If document is other size, please submit copy that is already adjusted in A4 size from its original and without staples or pasting pages.

 

Applicants are exempted from submitting (4) and (5) indicated above if they can submit old/valid passports with used Japan Visa.

 

It is applicant’s responsibility to ensure that he/she meet the requirements for the grant of a visa. Submission of supporting documents not listed above is encouraged (e.g. applicant’s economic or social ties with the Philippines, urgent reasons for visit: medical certificate, wedding invitation).

 

Passport must have a validity of more than six (6) months from date of departure.


 

Important Notice on New Restrictions related to Novel Coronavirus

1. Since the Government of Japan designated Novel Coronavirus(AKA COVID-19) as "Designated Infectious Disease" under the Infectious Diseases Act of Japan, foreign nationals who are deemed as patients of Novel Coronavirus shall be denied permission to land in Japan pursuant to Immigration Control and Refugee Recognition Act of Japan. As the Government of Japan also designated Novel Coronavirus as “Quarantinable Infectious Diseases” under Quarantine Act of Japan, foreign nationals who are suspected of being afflicted with Novel Coronavirus would be ordered to be quarantined with no exception for valid visa holders.
 
2. The Government of Japan also decided on 7 Mar.2020 that for the time being, those who fall under the three categories below are not permitted to land in Japan unless there are exceptional circumstances.

- Foreign nationals who have travelled to Hubei/Zhejiang Province in People's Republic of China, or Daegu/Gyeongsangbuk-do (Cheongdogun/ Gyeongsan-si/Andong-si/Yeongcheon-si/Chilgok-gun/Uiseonggun/ Seongju-gun/Gunwi-gun) in Korea, or Qom/Teheran/Gilan Provinse in Islamic Republic of Iran within 14 days before arriving in Japan.
- Foreign nationals who hold a passport issued by the Hubei/Zhejiang Province Authority.
- Foreign Nationals aboard a passenger ship which sails for the purpose of entering Japanese port and carries a risk of being
afflicted by an outbreak of Novel Coronavirus infectious disease.

3. Based on the “Principle Criteria of Visa Issuance”, visa applications of those who fall under the categories being not permitted to land in Japan are not accepted. Visa applicants are required to fill in and submit 
questionnaires on whether or not they have (or will have) travelled to Hubei/Zhejiang Province in People's Republic of China, or Daegu/Gyeongsangbuk-do (Cheongdo-gun/Gyeongsansi/ Andong-si/Yeongcheon-si/Chilgok-gun/Uiseong-gun/Seongjugun/ Gunwi-gun) in Korea, or Qom/Teheran/Gilan Provinse in Islamic Republic of Iran within 14 days before arriving in Japan.
 
4. 
Please note that even those foreign nationals who were already issued valid visa of Japan would not be permitted to land in Japan if they fall under the categories mentioned in 1 and 2 above.
 
5. 
Any false declaration in the questionnaires will be subject to refusal of visa issuance, and then new visa application will not be accepted for six months for the same purpose of visit. The visa will be made null and void when such false declaration is revealed after the visa issued.
 
6. 
Any false declaration upon landing in Japan may be subject to punitive measures including imprisonment, imposition of fine, revoking of residential status as well as deportation from Japan.
